You agree to indemnify and hold GLHP and its parents, affiliates, employees, officers, directors and agents harmless from any claim or demand, including reasonable attorneys' fees, made by a third party arising from any material you contribute to Message Boards or Chat Rooms you access via GLHP.com.

You understand that GLHP has no obligation to monitor Message Boards or Chat Rooms. GLHP reserves the right at all times to disclose any information regarding the Message Boards or Chat Rooms, or to remove any information or materials, in whole or in part, that in GLHPs' sole and absolute discretion are objectionable.

When using GLHP.com, please be certain that anything you disclose does not compromise your personal safety. Do not provide your name, phone number, social security number or any other personally identifying information to people you do not know. Do not continue any conversation online that makes you feel uncomfortable.

Limitation of Liability

You agree by accessing GLHP.com that under no circumstances or any theories of liability under international or civil, common or statutory law including but not limited to strict liability, negligence or other tort theories or contract, patent or copyright laws, will GLHP be liable for damages of any kind occurring from the use of this web site or any information, goods or services obtained on this web site including direct, indirect, consequential, incidental, or punitive damages (even if GLHP has been advised of the possibility of such damages), to the fullest extent permitted by law.

Minors and Child Online Privacy Protection Act

GLHP.com does not direct content to Minors and does not intend to collect personal information from Minors and therefore is not subject to the Child Online Privacy Protection Act. If you allow your minor child, or a child for whom you are legal guardian (a "Minor"), to access and use GLHP.com, you agree that you will be solely responsible for: (i) the online conduct of such Minor; (ii) monitoring such Minor's access to and use of GLHP.com; and (iii) the consequences of any use.

Great Lakes Health Plan’s Privacy Policy

When visiting GLHP.com, no information is collected and stored about you. Any information that you submit to this web site is not retained and is used solely to respond to your inquiry. Your privacy is important to GLHP. GLHP does not sell any personal information about you. GLHP does not disclose any personal information about you without your written consent unless we are allowed by law to do so.

To serve you, GLHP must collect, use, and disclose certain personal information about you. GLHP receives personal information about you from the following sources:

• Medicaid enrollment lists, which have information about your age, sex, identifying number and other personal information;

• Reports about the cost and quality of the health care services you receive, bills for healthcare services you receive,

• Complaints and grievances about your care and information necessary for coordination of benefits with any other health care coverage you may have.

GLHP does not collect such personal information through your use of this web site. The personal information is collected only through GLHP's administration of its Medicaid benefit program as permitted by federal and state law. As part of GLHP's contractual and administrative duties, it does disclose your personal information as allowed by law. GLHP may disclose personal information about you to pay claims for the care you receive, to deal with complaints and grievances about your care, to address quality of care or utilization management issues associated with services furnished by providers, to provide case and disease management services, to coordinate your health care benefits with any other health care coverage you may have and to provide information required by government and accreditation agencies.

GLHP policy is to allow personal information about you to be seen only by people who have a need and a legal right to see that information. We maintain safeguards for your personal information. GLHP regularly reviews these safeguards to make sure that your privacy is protected.

Personal Identifying Information

GLHP will not request any personally identifying information from you in order to use our web site.

Access

Since GLHP.com does not collect any information through administration of this web site, we do not know who you are and there is no information stored by this web site for you to access.

Security

Any transmission that you make on the Internet is not secure. However, since we do not collect any information, we have no need for security beyond that necessary to maintain this web site.

Changes

We will post any substantial changes in this privacy policy at least 30 days prior to the change taking effect. Café au lait spot: A flat spot on the skin that is the color of coffee with milk (café au lait) in persons with light skin, or a darker appearance (café noir) than the surrounding skin in persons with dark skin. About 10% of the general population have café au lait spots, and they can be removed with a Yag laser. Café au lait spots are in themselves harmless, but in some cases they may be a sign of neurofibromatosis. The presence of 6 or more café au lait spots each of which is 1.5 centimeters or more in diameter is diagnostic of neurofibromatosis. Conversely, most people with neurofibromatosis who are at least 5 years old have 6 or more café au lait spots 1.5 centimeters or more in diameter.
